So what I've done is I've put my kit together in a way so that when I arrive at set I can gear up with what I'll need and leave the other stuff at my base camp. (car, hotel room, or trailer). Since I've been doing lots of day shoots around LA I have a 12 gallon container and a small dolly to cart my stuff around. It's pretty handy and I can use the container as a make shift table if needed.
I also have found it handy to have a bag of clothes in the trunk as well. Sometimes if your working all day outside you need to bundle up at the end of the day.
It may seem like a lot of stuff (there is a lot of small stuff), but once I weed out what I need for the day I don't have that much and I can carry everything I need on me.
